Account Applications and Changes

All account applications and changes must be carried out by completing one or more of the application forms available below for download.

Which form do I need?

Form ED1

This form is needed to apply for an edischarges account. (Please refer to the technical requirements before applying for an account)

Form ED2

This form is used to change existing account details such as contact details, administrator details, IP or email address.

Form ED3

This form, to be completed by the account administrator is used to setup account users and their associated roles and user rights.

Form ED4

This form, to be completed by the account administrator is used to request changes to existing user accounts. This form is used to make changes to the user role, to request a new user password or PAN and to update a user's contact details. It is also used to notify us when a user is to be removed from the account.
